AbstractAs the fact, the petroleum energy has not been completely substituted by any other energy up till now. Therefore, the petroleum energy is still the main energy of the world's infrastructure, currently world's demand is still increasing. Moreover, many countries are conducting more exploration activities in order to discover more oil or gas reserves and to provide the products to their countries or for trading purpose. As the result of the increasing of petroleum demands, a lot of offshore structures have been fabricated and installed in order to produce more petroleum and to satisfy the needs to the citizen. However, the fabrication of offshore structure has been usually put as the significant activity because of a high consequence of its hazard and risk exposure. Moreover, offshore structure fabrication activities are working with heavy machines and equipments which usually exposed with hidden risks. Therefore, several safety methods have been conducted in order to reduce the mentioned problems. Currently, risk assessment method is widely applied in industries including oil and gas industry. Consequently, this study will majorly spotlight on risk assessment based upon HAZID, probability of occurrence, consequence of severity and so on. Furthermore, this study have been conducting by utilizing fault tree analysis (FT A) as a tool for analyzing the root causes of failure in offshore structure fabrication activities. After the completion of this study, the results showed the risk level of each hazard (Resulted from probability of occurrence and consequence of severity), precaution & mitigation plans for risk controlling, root causes of failure and the probability of overall project failure. Considering the result of this study, numerous identified hazards possess a high level of risk that the industries should take into consideration in order to reduce the number of hazards occurred in each activities. The result is expected to contribute to all stakeholders and also researcher in order to develop a risk assessment guideline.
